How to Create an Advanced Medical Directive (AMD) in Singapore - A step-by-step guide
An Advance Medical Directive (AMD) allows you to legally record your wish to refuse extraordinary life-sustaining treatment when experiencing terminal illness and loss of capacity. Follow this guide to ensure your directive is valid and properly registered in Singapore.
1. Confirm Eligibility (Age & Mental Capacity)
You can make an AMD if you are:
- Aged 21 or older
- of sound mind and are able to understand and make informed healthcare decisions
2. Obtain the Official AMD Form
Download the Form 1 Advanced Medical Directive from the Ministry of Health (MOH) or pick it up at polyclinics and participating clinics. the form is a single-sheet legal document.
3. Complete the Form
Fill the form with your personal data and declaration stating that, under terminal illness and when unable to decide, you do not want extraodinary life-sustaining treatment
4. Sign with Two Witnesses
Sign the AMD form in front of:
- A registered medical practitioner
- A second adult witness ( 21 Years old and above), who must not benefit from your death
- Your age and sound mind
- That the directive is voluntary
5. Register with the Ministry of Health
Place the form in a sealed envelope and submit it by mail or hand delivery to the Registry of Advance Medical Directive, MOH. Faxed copies are not accpeted and the directive only takes effect once officially registered.
6. Understand When It Applies
Your AMD will be activated only if:
- You suffer from a terminal illness (irreversible,death imminent).
- You are inconscious or lack capacity to communicate your medical wishes.
- Three doctors including 2 specalists confirm these conditions.
If the first 3 doctors disagree, a panel of 3 specalists appointed by MOH will judge whether the criterias are met.
7. Revoke or Update Your Directive
You may revoke or update your AMD at any time as long as you are of sound mind. This requires at least one adult witness and formally submited to the Registar.
